ABSTRACT

Introducción: Las comunidades de macroinvertebrados son afectadas simultáneamente por la calidad del agua y las características físicas del hábitat acuático, complicando su uso en la bioindicación. Objetivo: Determinar cuáles variables del hábitat condicionan la comunidad de macroinvertebrados acuáticos en algunas corrientes (quebradas) de montaña del Oriente antioqueño (Colombia). Métodos: El muestreo se realizó en febrero 2021 (periodo de transición seco-lluvia), para evaluar variables físicas y químicas en tres tipos de mesohábitats: rápidos, rizos y pozas en corrientes con coberturas vegetales contrastantes. Los macroinvertebrados fueron recolectados en diez sitios de muestreo con red tipo net, pantalla y manual, y preservados en etanol al 70 %. Resultados: Se recolectaron 4 484 macroinvertebrados (16 órdenes, 46 familias y 75 géneros). El mesohábitat rizo presentó mayores valores de diversidad y abundancia, mientras las pozas presentaron los menores. Hubo diferencias en la concentración de oxígeno, profundidad, velocidad y abundancia de macroinvertebrados entre mesohábitats. Las pozas defirieron de los otros mesohábitats en profundidad, velocidad, así como en la composición, abundancia y riqueza de macroinvertebrados, y fue el hábitat de menor preferencia. Conclusión: La velocidad, profundidad y concentración de oxígeno disuelto, desempeñan un papel muy importante en el establecimiento de las comunidades de macroinvertebrados en los diferentes mesohábitats. En el mismo tipo de mesohábitat, la calidad de la cobertura vegetal determinó la diversidad y abundancia de esta comunidad.


Introduction: Macroinvertebrate communities are affected by water quality and physical characteristics of the aquatic habitat, simultaneously, complicating their use as bioindicators. Objective: To determine which habitat variables regulate the macroinvertebrate community in mountain streams in Eastern of Antioquia (Colombia). Methods: Sampling was carried out in February 2021 (dry-rain transition period), to evaluate physical and chemical variables in three types of mesohabitat: ripples, pools, and rapids in streams with contrasting vegetation covers. The macroinvertebrates were collected from ten sampling sites with a net, screen and manual type net preserved with 70 % ethanol. Results: 4 484 macroinvertebrates were collected (16 orders, 46 families and 75 genera). The ripples mesohabitat presented higher values of diversity and abundance, while the pools presented the lowest. There were differences for oxygen concentration, depth, speed, and macroinvertebrate abundance between mesohabitats. Pools differed from the other mesohabitats in depth, speed, as well as in composition, abundance, and richness in macroinvertebrates, and was the least preferred mesohabitat. Conclusion: Speed, depth, dissolved oxygen concentration played a very important role in the establishment of macroinvertebrates community in different mesohabitats. For the same type of mesohabitat, the quality of the plant cover determined both diversity and abundance of this community.

ABSTRACT

Objective:To evaluate the efficacy of disposable disinfection cap in preventing catheter-related blood stream infections (CRBSIs).Methods:Literature on the prevention of CRBSIs by disposable disinfection caps were retrieved from CNKI, Wanfang Database, VIP database, Chinese Biomedical Literature Database, PubMed, Cochrane Library, Embase and Web of Science databases. The retrieval period was from the database construction to June 30, 2022. After literature screening, data extraction and quality evaluation were independently carried out by 2 researchers, RevMan5.4 software was used for analysis.Results:A total of 12 articles were included, including 9 832 patients. Meta-analysis results showed that compared with conventional manual disinfection, disposable disinfection cap could reduce the incidence of CRBSIs, and the difference was statistically significant ( RR = 0.58, 95% CI 0.43-0.79, P<0.01). Disposable disinfection cap could reduce the incidence of CRBSIs in adults, but there was no significant difference in the incidence of CRBSIs in children ( P>0.05). It could reduce the incidence of CRBSIs in patients with indwelling vascular catheters in ICU ( RR = 0.58, 95% CI 0.38-0.89, P<0.05), but there was no statistical difference in the incidence of CRBSIs in patients without indwelling vascular catheters in ICU ( P>0.05). In addition, the compliance of the use of disinfection caps by nursing staff was improved by 80% - 90% and the treatment cost was saved by about 282 - 464 dollars. Conclusions:Disposable disinfection caps can reduce the occurrence of CRBSIs, improve the disinfection compliance of nursing staff, and save the hospitalization cost of patients. It is suggested to be popularized in clinical practice.

ABSTRACT

Blood stream infection (BSI),a blood-borne disease caused by microorganisms such as bacteria,fungi,and viruses,can lead to bacteremia,sepsis,and infectious shock,posing a serious threat to human life and health.Identifying the pathogen is central to the precise treatment of BSI.Traditional blood culture is the gold standard for pathogen identification,while it has limitations in clinical practice due to the long time consumption,production of false negative results,etc.Nanopore sequencing,as a new generation of sequencing technology,can rapidly detect pathogens,drug resistance genes,and virulence genes for the optimization of clinical treatment.This paper reviews the current status of nanopore sequencing technology in the diagnosis of BSI.

ABSTRACT

Objective:To report the results of national surveillance on the distribution and antimicrobial resistance profile of clinical bacterial isolates from bloodstream infections in China in 2021.Methods:The clinical bacterial strains isolated from blood culture from member hospitals of Blood Bacterial Resistant Investigation Collaborative System (BRICS) were collected during January 2021 to December 2021. Antibiotic susceptibility tests were conducted by agar dilution or broth dilution methods recommended by Clinical Laboratory Standards Institute (CLSI). WHONET 5.6 was used to analyze data.Results:During the study period, 11 013 bacterial strains were collected from 51 hospitals, of which 2 782 (25.3%) were Gram-positive bacteria and 8 231 (74.7%) were Gram-negative bacteria. The top 10 bacterial species were Escherichia coli (37.6%), Klebsiella pneumoniae (18.9%), Staphylococcus aureus (9.8%), coagulase-negative Staphylococci (6.3%), Pseudomonas aeruginosa (3.6%), Enterococcus faecium (3.6%), Acinetobacter baumannii (2.8%), Enterococcus faecalis (2.7%), Enterobacter cloacae (2.5%) and Klebsiella spp (2.1%). The prevalence of methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-resistant coagulase-negative Staphylococcus aureus were 25.3% and 76.8%, respectively. No glycopeptide- and daptomycin-resistant Staphylococci was detected; more than 95.0% of Staphylococcus aureus were sensitive to ceftobiprole. No vancomycin-resistant Enterococci strains were detected. The rates of extended spectrum B-lactamase (ESBL)-producing isolated in Escherichia coli, Klebsiella pneumoniae and Proteus mirabilis were 49.6%, 25.5% and 39.0%, respectively. The prevalence rates of carbapenem-resistance in Escherichia coli and Klebsiella pneumoniae were 2.2% and 15.8%, respectively; 7.9% of carbapenem-resistant Klebsiella pneumoniae was resistant to ceftazidime/avibactam combination. Ceftobiprole demonstrated excellent activity against non-ESBL-producing Escherichia coli and Klebsiella pneumoniae. Aztreonam/avibactam was highly active against carbapenem-resistant Escherichia coli and Klebsiella pneumoniae. The prevalence rate of carbapenem-resistance in Acinetobacter baumannii was 60.0%, while polymyxin and tigecycline showed good activity against Acinetobacter baumannii (5.5% and 4.5%). The prevalence of carbapenem-resistance in Pseudomonas aeruginosa was 18.9%. Conclusions:The BRICS surveillance results in 2021 shows that the main pathogens of blood stream infection in China are gram-negative bacteria, in which Escherichia coli is the most common. The MRSA incidence shows a further decreasing trend in China and the overall prevalence of vancomycin-resistant Enterococci is low. The prevalence of Carbapenem-resistant Klebsiella pneumoniae is still on a high level, but the trend is downwards.

ABSTRACT

Aims: Reliance on unhealthy water sources such as hand dug wells and streams have led to the occurrence of waterborne diseases such as amoebic dysentery, gastroenteritis, typhoid fever and cholera among several communities in Rivers State. This study tends to identify selected multidrug resistant pathogens isolated from streams and wells water sources in three Local Government Areas of Rivers State using molecular techniques. Methodology: A total of 48 well and stream water samples were collected aseptically from 24 sampling sites in Ikwerre, Emohua and Etche Local Government Areas of Rivers State. Samples were analysed using membrane filtration technique for total heterotrophic bacterial count and total coliform count. The well and stream water samples were also cultured for E. coli, Salmonella and Vibrio sp. Antibiotic sensitivity test of the isolates was carried out using the disk diffusion method and multidrug resistant isolates were identified using PCR-based techniques (16S rRNA). Results: The results of this study showed that total aerobic heterotrophic bacteria count occurred more in Emohua LGA for both well and stream water sources with bacterial count of 1.71 x104 cfu/ml for well water samples and 2.35 x 104 cfu/ml for stream water samples. The mean fungal count of the stream water samples was highest in Emohua LGA with 2.55 x 104 cfu/ml followed by Ikwerre LGA which had 4.20 x 103 cfu/ml and Etche LGA with the least count of 1.80 x 102 cfu/ml. All the isolated bacteria species were found to be resistant to more than 50% of the antibiotics used with highest Multi Antibiotic Resistant Index of 0.625. multidrug resistant bacterial isolates identified include Vibrio sp strain 201707CJKOP-Y162 (MG593726), Bacillus sbtilis clone N55 (JQ622582), Chryseobacterium sp strain CB2915-325-DE_0621 (MH512534), Enterobacter sp strain AB55 (MF407145), Klebsiella pneumonia strain M792-16(MH680830), Shigella sonnei strain AR_0030(CP032523), Escherichia coli strain WECHEC025943(CP027205) and Salmonella bongori strain KC153129.116 (MG663494). Conclusion: Access to good quality or potable drinking water and efficient sanitary practices are fundamental to human health and economic development. The occurrence of multidrug resistant pathogenic bacterial isolates in natural water sources requires serious routine evaluation in order to forestall the outbreak of waterborne disease epidemics.

La resistencia a los antimicrobianos es una de las preocupaciones más importantes a nivel mundial, ya que determina un aumento de la morbimortalidad de los pacientes y el incremento de los costos de la atención de salud; problema cuyo abordaje debe ser realizado bajo el enfoque de Una Salud. El objetivo principal de este trabajo fue evaluar la resistencia antimicrobiana en Escherichia coli provenientes de muestras cecales de bovinos de carne faenados en frigoríficos de la zona del Arroyo Mburicao, Asunción-Paraguay, de setiembre a noviembre de 2021. Fueron colectadas 181 muestras de ganados provenientes de la Región Oriental y Occidental, y cultivadas en agar cromogénico suplementados con ciprofloxacina, colistina, cefotaxima y meropenem. Las colonias aisladas fueron identificadas, genotipificadas (PCR convencional) y sometidas a pruebas de susceptibilidad. El principal hallazgo fue la confirmación de la resistencia a las fluoroquinolonas en un 8,3% debida a la portación de los genes Qnr S, Qnr B y aac-6´-Ib-cr, involucrados en el mecanismo plasmídico de resistencia a quinolonas. Además, la resistencia acompañante encontrada en los aislamientos resistentes del 100% a tetraciclina y 53% a trimetoprim/sulfametoxazol. No se encontraron aislamientos con resistencia a cefotaxima, colistina y meropenem. Los resultados obtenidos son de suma relevancia para la generacion de conocimiento sobre los perfiles de resistencia de microorganismos en el sector veterinario, pudiendo contribuir a la elaboración de guías nacionales para el uso prudente de antimicrobianos y apoyando al trabajo multisectorial en la lucha para la contención de la resistencia antimicrobiana.


Antimicrobial resistance is one of the most critical concerns worldwide since it determines an increase in patient morbidity and mortality and the increase in healthcare costs, a problem whose approach must be carried out under the One Health approach. The main objective of this work was to evaluate antimicrobial resistance in Escherichia coli from cecal samples of cattle for meat slaughtered in meat processing plants in the Arroyo Mburicao area, Asunción-Paraguay, from September to November 2021. One hundred eighty one samples of cattle from the Eastern and Western Region were collected, and cultured on chromogenic agar supplemented with ciprofloxacin, colistin, cefotaxime, and meropenem. Isolated colonies were identified, genotyped (conventional PCR) and subjected to susceptibility tests. The main finding was the confirmation of resistance to fluoroquinolones in 8.3% due to the carrying of genes Qnr S, Qnr B and aac-6'-Ib-cr, involved in the plasmid mechanism of resistance to quinolones. In addition, the accompanying resistance found in the isolates was 100% resistant to tetracycline and 53% to trimethoprim/sulfamethoxazole. Isolates with resistance to cefotaxime, colistin and meropenem were not found. The results obtained are highly relevant for the generation of knowledge about the resistance profiles of microorganisms in the veterinary sector. It can contribute to the development of national therapeutic guidelines for the prudent use of antimicrobials, and supporting multisectoral work in the fight to contain antimicrobial resistance.

Introduction: Blood stream infection are very common in the pediatric age group and these are one of the common causes of morbidity and mortality in children. In developing countries ,the rate of blood stream infection in children is about 20-50%.The present study was undertaken to determine the etiological agents causing blood stream infection and their antibiotic susceptibility pattern in pediatric patients. Patients with bacteremia may have either a transient bacteremia or persistent bacteremia which can be self-limited without development of focal infection or sequelae or may progress to a more serious fatal infection or toxic effects. The present study in a hospital basedMethodology: single centred, Observational study, of 3 years. Blood sample were collected in BacTec bottle and standard microbiological protocol were applied for the isolation identification of bacteria strains. Antimicrobial susceptibility test was performed by the Kirby Bauer Disc Diffusion Method as per CLSI 2019 guidelines. Out of total 350 bloodResults: samples received for culture,87(23.14%)were culture positive, out of which 42/87(48.27%) were Gram positive organisms and 39/87(44.82%) were Gram-negative organisms and 6 /87(6.89%) were candida spp. The most common organism was Staphylococcus aureus(31.03%) the predominant organism followed by Klebsiella pneumoniae(21.83%) and Streptococcus pneumoniae (9.19%), Escherichia coli, Enterobacter cloacae each (5.74%). All Gram positive bacteria were susceptibile to vancomycin, teicoplanin and linezolid. 11/27(40.74% )of Staphylococcus aureus were Methicillin resistant Staphylococcus aureus (MRSA) strains. All Gram negative bacteria were susceptibility to amikacin, Colistin,Tigecycline. Staphylococcus aureus is the leading cause ofConclusion: childhood septicemia in this locale, has been decline in susceptibility of the pathogens to common antibiotics which ultimately stresses on the need for continuous screening and surveillance for antibiotic resistance in the pediatric ward and calls for increased efforts to ensure more rational use of these drugs.

ABSTRACT

Background: Bloodstream infections (BSIs) account for large-scale morbidity and mortality among cancer patients requiring a rational antibiotic policy. In Bangladesh, there is a paucity of data regarding incidence and pattern of BSI in such patients. Objective:To evaluate the pattern of BSI in cancer patients and their sensitivity and resistance toward antibiotic.Material & Methods:The present study was a retrospective analysis of BSI patterns among various cancer patients treated at Department of Oncology, Enam Medical College Hospital, Saver, Bangladesh during the period from January to June 2021. Blood culture results and their sensitivity patterns of these cancer patients along with the demographic characteristics were collected from the records and maintained in the structured pro forma. Before starting empirical antibiotics, 10 ml of blood was collected into Bract/ALERT culture media.Results:A total of 82 patients/episodes had confirmed BSI. Gram-negative bacteria accounted for 43 (52.4%) cases, followed by Gram positive 38 (46.4%) cases and 1 case of candida species. The most common organisms isolated were Klebsiella pneumonia and Staphylococcus aureus consisting of 17 cases each. The Gram-negative bacterial isolates (n = 43) were sensitive to cefoperazone plus sulbactam, piperacillin plus tazobactam, carbapenem, and colitis in 18 (41.9%), 19 (44.2%), 29 (67.4%), and 40 (93%) episodes, respectively. The sensitivity of Gram-positive bacteria (n = 38) to vancomycin, linezolid, and teicoplanin was seen in 37 (97.3%), 37 (97.3%), and 35 (92.1%) episodes, respectively. Multidrug-resistant bacteria accounted for 17 (39.5%) cases of Gram-negative isolates and 9 (53%) of which were K. pneumonia. Extended spectrum beta-lactamase activity was seen in 11 of 26 episodes of Enterobacteriaceae. Four of 17 S. aureus and 3 of 11 coagulase-negative Staphylococci were methicillin resistant, and 1 of 2 cases of Enterococcus was vancomycin resistant.Conclusions:Gram-negative bacteria are the predominant cause of BSI in cancer patients and development of a high degree of resistance to commonly used antibiotics is challenging.

Purpose: To evaluate the causes of jet stream injury (JSI)?related iatrogenic retinal breaks (IRBs) during vitreoretinal surgery (VRS). Methods: The precise surgical environment, which includes the indication and type of surgical procedure, retina status, details of instrumentation and fluidic parameters, and characteristics of the jet responsible for the IRB, was noted from case records. The nature of IRB and its healing and impact on anatomical and visual outcomes were analyzed. Results: Five eyes of five patients with complete documentation of both the JSI and the IRB were included. Two cases were operated for macular hole, and one each for vitreous hemorrhage, retinal detachment, and endophthalmitis. One case had infusion?fluid?related JSI, while four developed it because of injection of surgical adjuncts (drugs, PFCL, and dye). JSI developed in two cases when the vitreous cavity was filled with fluid, while it was air?filled in three cases. In four cases, the fluid migrated into subretinal space, necessitating further maneuvers following which the breaks healed, but were directly responsible for vision loss in two cases. Conclusion: JSI related IRBs are rare but may be directly responsible for vision loss if they impact the macula. The balance between jet stream velocity, its distance from the retinal surface, the intervening media (vitreous cavity), and retinal health play an important role. It can occur because of both infusion as well as injection jets. Precautions must be taken in cases vulnerable to complications with suggested modifications in the surgical technique.

The Cautín River is closely related with the economic development of Temuco city, (38°S; Chile). Existing knowledge of the Cautín River is limited to information about its biological characteristics as a reference for the evaluation and assessment of water quality. The object of this study was to develop taxonomic characterisation of the benthic macroinvertebrates along the main course of the Cautín River, and to study the community structure using correlation analysis between community parameters. To carry out this research, the macroinvertebrate community was studied in 10 sampling sites distributed along the main course of the river. The samples were taken in summer (1997 and 2000), when optimal hydrological conditions existed. Analysis of the samples showed that the benthic fauna was composed of 56 taxa, the dominant group being insects with 48 taxa. Three main sectors were recognized in the course of the Cautín River: high, middle and low. Each sector has restricted-distribution species, while other species are widely distributed along the river. These distribution patterns seem to be influenced by dissolved oxygen concentration, temperature, altitudinal distribution and anthropo-cultural activity, present at every sampling site. Finally, this research provides a first approach to the biology of the Cautín River. Further studies could be planned on the basis of this knowledge to investigate water quality indicators based on macroinvertebrate communities.


O rio Cautín está intimamente relacionado ao desenvolvimento econômico da cidade de Temuco (38°S; Chile). Quanto ao conhecimento total do rio Cautín, existem informações limitadas sobre as características biológicas que podem servir de referência para a avaliação da qualidade da água. Este estudo tem o objetivo de caracterizar os macroinvertebrados bentônicos taxonomicamente ao longo do curso principal do rio Cautín e estudar a estrutura da comunidade usando análise de correlação entre os parâmetros dela. Para realizar esta pesquisa, a comunidade de macroinvertebrados foi estudada em dez locais de estudo distribuídos ao longo do rio principal. As amostras foram coletadas no verão (1997 e 2000), em razão das condições hidrológicas ideais. A análise das amostras mostrou que a fauna bentônica é composta de 56 táxons, sendo o grupo dominante o de insetos com 48 táxons. Na distribuição do principal no curso do rio, três setores são reconhecidos: alto, médio e baixo no rio Cautín. Cada setor possui espécies de distribuição restrita e outro com ampla distribuição ao longo do rio. Esses padrões de distribuição parecem influenciar a concentração de oxigênio dissolvido, a temperatura, a distribuição altitudinal e a atividade antropocultural desenvolvida em todos os locais de amostragem. Finalmente, esta pesquisa fornece uma primeira abordagem biológica do rio Cautín e, de acordo com esses conhecimentos, um estudo posterior pode ser planejado em relação aos indicadores de qualidade da água e com base nas comunidades de macroinvertebrados.

Finalmente, esta pesquisa fornece uma primeira abordagem biológica do rio Cautín e, de acordo com esses conhecimentos, um estudo posterior pode ser planejado em relação aos indicadores de qualidade da água e com base nas comunidades de macroinvertebrados.

ABSTRACT

Abstract The Cautín River is closely related with the economic development of Temuco city, (38°S; Chile). Existing knowledge of the Cautín River is limited to information about its biological characteristics as a reference for the evaluation and assessment of water quality. The object of this study was to develop taxonomic characterisation of the benthic macroinvertebrates along the main course of the Cautín River, and to study the community structure using correlation analysis between community parameters. To carry out this research, the macroinvertebrate community was studied in 10 sampling sites distributed along the main course of the river. The samples were taken in summer (1997 and 2000), when optimal hydrological conditions existed. Analysis of the samples showed that the benthic fauna was composed of 56 taxa, the dominant group being insects with 48 taxa. Three main sectors were recognised in the course of the Cautín River: high, middle and low. Each sector has restricted-distribution species, while other species are widely distributed along the river. These distribution patterns seem to be influenced by dissolved oxygen concentration, temperature, altitudinal distribution and anthropo-cultural activity, present at every sampling site. Finally, this research provides a first approach to the biology of the Cautín River. Further studies could be planned on the basis of this knowledge to investigate water quality indicators based on macroinvertebrate communities.


Resumo O rio Cautín está intimamente relacionado ao desenvolvimento econômico da cidade de Temuco (38°S; Chile). Quanto ao conhecimento total do rio Cautín, existem informações limitadas sobre as características biológicas que podem servir de referência para a avaliação da qualidade da água. Este estudo tem o objetivo de caracterizar os macroinvertebrados bentônicos taxonomicamente ao longo do curso principal do rio Cautín e estudar a estrutura da comunidade usando análise de correlação entre os parâmetros dela. Para realizar esta pesquisa, a comunidade de macroinvertebrados foi estudada em dez locais de estudo distribuídos ao longo do rio principal. As amostras foram coletadas no verão (1997 e 2000), em razão das condições hidrológicas ideais. A análise das amostras mostrou que a fauna bentônica é composta de 56 táxons, sendo o grupo dominante o de insetos com 48 táxons. Na distribuição do principal no curso do rio, três setores são reconhecidos: alto, médio e baixo no rio Cautín. Cada setor possui espécies de distribuição restrita e outro com ampla distribuição ao longo do rio. Esses padrões de distribuição parecem influenciar a concentração de oxigênio dissolvido, a temperatura, a distribuição altitudinal e a atividade antropocultural desenvolvida em todos os locais de amostragem. Finalmente, esta pesquisa fornece uma primeira abordagem biológica do rio Cautín e, de acordo com esses conhecimentos, um estudo posterior pode ser planejado em relação aos indicadores de qualidade da água e com base nas comunidades de macroinvertebrados.

ABSTRACT

@#Abstract: To analyze the clinical, therapeutic and laboratory characteristics of disseminated cryptococcosis caused by Cryptococcus neoformans invading the blood stream in patient with liver cirrhosis and splenectomy. A 30-year-old male underwent splenectomy plus pericardial devascularization due to "splenomegaly and hypersplenism" in March in 2016. The patient had intermittent fever after operation for many times, and successively accompanied with back pain, left lower limb abscess and right hip pain. The highest body temperature was 39 ℃. CT and MRI revealed the lung lesion and multiple bone destruction. During that period, the effect of antibiotics was not good. On April 19th, 2017, Gram's stain, India ink stain, API 32C, Vitek 2 Compact, ribosomal ITS and IGS sequence analysis were performed to identify the strain isolated from the pus and blood stream. The serum of the patient was detected for cryptococcal antigen. Antifungal susceptibility test was used to determine drug sensitivity and minimum inhibitory concentration (MIC). The Cryptococcus neoformans isolated from fresh pus specimen showed a prominent, thick capsule after India ink stain. The colonies isolated from pus and blood stream were identified Cryptococcus neoformans using API 32C, Vitek 2 Compact, and sequence analysis of rDNA ITS and IGS. Cryptococcal capsule antigen was positive. The minimal inhibitory concentrations of 5-Flucytosine, amphotericin B, fluconazole, itriconazole, voriconazole against the isolate were <4 μg/mL, <0.5 μg/mL, 4 μg/mL, ≤0.25 μg/mL, 0.125 μg/mL respectively. The patient was initially treated with intravenous amphotericin B and flucytosine. After anti-Cryptococcus treatment for two months, the patient clinically improved, and the lesions were reduced on a follow-up CT scan. The patient made a full functional recovery after treatment for six months. Cryptococcosis has hidden onset, atypical clinical symptoms and lack of specificity. Blood stream is the main channel for Cryptococcus to spread and involve many organs of the whole body, including skin, bone and so on. Therefore, early use of blood culture to monitor blood flow dissemination, actively removing the primary focus and cutting off the infection route in time and carrying out effective anti-Cryptococcus treatment are conducive to the patient's early recovery.

ABSTRACT

Objective:To investigate the characteristics of distribution and drug resistance of urinary bacteria in the mid-stream urine of patients with infectious stones.Methods:The retrospective study analyzed the clinical data of 254 patients with infectious stones in the First Affiliated Hospital of Guangzhou Medical University from September 2016 to September 2018. All patients were treated with PCNL. Overall, there were 101 male and 153 female patients, with the mean age of(51.5±12.3) years, and the mean stone burden of 1443.5(660.8, 2837.5) mm2. There were 58 (22.8%) patients with hypertension, 17(6.7%) patients with diabetes and 195(76.8%)with hydronephrosis. The mid-stream urine samples were obtained for bacterial culture and susceptibility test, and the results of urine culture and antimicrobial susceptibility were recorded and analyzed.Results:Of 254 patients involved in this study, 89(35.0%) were positive and 165 (65.0%) were negative for urinary bacterial culture of the mid-stream. The proportion of patients with positive urine bacterial culture of the mid-stream who had positive urine leucocytes, positive urine nitrite and postoperative pyrexia were 86.5%(77/89), 64.0%(57/89), 25.8%(23/89), respectively, which was higher than that of patients with negative urine bacterial culture of the mid-stream [50.3%(83/165), 14.5%(24/165), 14.5%(24/165), P<0.05]. Four teen kinds of bacteria were detected from the mid-stream urine, and the three bacteria with the highest detection rate in turn were Escherichia coli of 38.2%(34/89), Proteus mirabilis of 15.7%(14/89), and Pseudomonas aeruginosa of 11.2%(10/89). The results of this study showed that three common bacteria had high resistance to drug including Cefazolin, Cefuroxime, Cefuroxime ester, Ampicillin and Co-trimoxazole (all resistance rate>40%). The resistance rates of Escherichia coli and Pseudomonas aeruginosa to Ciprofloxacin and Levofloxacin were higher than or equal to 40%. The resistance rates of Escherichia coli and Proteus mirabilis to meropenem, imipenem, ertapenem, piperacillin/tazobactam and amikacin were all lower than 10%. In addition, the resistance rates of Escherichia coli to nitrofurantoin and tigecycline and Proteus mirabilis to tobramycin, aztreonam and cefoxitin were all less than 10%. The resistance rates of Pseudomonas aeruginosa to ceftazidime, cefepime, gentamicin and aztreonam were less than 10%. Conclusions:The highest detection rate of urinary bacteria in culture of the mid-stream with infectious stones are Escherichia coli, Proteus mirabilis and Pseudomonas aeruginosa, all of which showed high resistance to Ampicillin, Cotrimoxazole, and some cephalosporins. Escherichia coli and Pseudomonas aeruginosa showe high resistance to Ciprofloxacin and Levofloxacin, and all of the three bacteria have low resistance rates to some β-Lactamase inhibitor complex and carbapenems, suggesting a reference for clinical empirical medical treatment.

RESUMEN La metodología mapa de cadena de valor se ha convertido en una herramienta fundamental para los gerentes debido a su capacidad de diagnosticar procesos empresariales, donde se identifican las actividades que no agregan valor y se proponen alternativas de mejora basadas en distintas técnicas Lean Manufacturing. En el presente artículo, se aplica la herramienta mapa de cadena de valor en los procesos de cosecha y postcosecha de una empresa que se dedica al cultivo y comercialización de piña para el mercado nacional e internacional. Al final de la investigación se identifican desperdicios dentro del proceso como el exceso de manipulación, la falta de capacitación al personal y la ausencia de planes eficientes de mantenimiento de maquinaria. A partir de estas debilidades, se logra definir un plan de acción para la empresa, el cual es simulado utilizando el software Flexsim, donde se contrasta los escenarios "antes" y "después" de la operación con el fin de ilustrar a los gerentes, los beneficios potenciales de la implementación de las propuestas de mejora, en donde se logra establecer una reducción del tiempo de ciclo en aproximadamente un 19% lo que permite aumentar los volúmenes de producción y cumplir con la demanda del cliente final.


ABSTRACT The Value Stream Mapping methodology has become a fundamental tool for managers due to its ability to diagnose business processes, where activities that do not add value are identified and improvement alternatives based on different Lean Manufacturing techniques are proposed. In this article, the Value Stream Mapping tool is applied in the harvesting and postharvest processes of a company that is dedicated to the cultivation and sale of pineapple for the national and international market. At the end of the investigation, waste is identified within the process, such as excessive handling, lack of training for personnel and the absence of efficient machinery maintenance plans. From these weaknesses, it is possible to define an action plan for the company which is simulated using the Flexsim software, where the "before" and "after" scenarios of the operation are contrasted in order to illustrate to the managers, the potential benefits of the implementation of the improvement proposals, where it is possible to establish a reduction in cycle time by approximately 19%, which allows increasing production volumes and meeting the demand of the end customer.


RESUMO A metodologia do Mapeamento do Fluxo de Valor tornou-se uma ferramenta fundamental para os gestores devido a sua capacidade de diagnosticar processos de negócios, onde atividades que não agregam valor são identificadas e alternativas de melhoria são propostas com base em diferentes técnicas de Lean Manufacturing. Neste artigo, a ferramenta de Mapeamento do Fluxo de Valor é aplicada aos processos de colheita e pós-colheita de uma empresa dedicada ao cultivo e comercialização do abacaxi para o mercado nacional e internacional. Ao final da pesquisa, foram identificados resíduos dentro do processo, tais como manuseio excessivo, falta de treinamento de pessoal e a ausência de planos eficientes de manutenção de máquinas. A partir destes pontos fracos, é possível definir um plano de ação para a empresa, que é simulado utilizando o software FlexSim, que contrasta os cenários "antes" e "depois" da operação a fim de ilustrar aos gestores, os benefícios potenciais da implementação das melhorias propostas, onde é possível estabelecer uma redução no tempo de ciclo em aproximadamente 19%, o que permite aumentar os volumes de produção e atender a demanda do cliente final.

Abstract Introduction One of the major cues that help in auditory stream segregation is spectral profiling. Musicians are trained to perceive a fine structural variation in the acoustic stimuli and have enhanced temporal perception and speech perception in noise. Objective To analyze the differences in spectral profile thresholds in musicians and nonmusicians. Methods The spectral profile analysis threshold was compared between 2 groups (musicians and nonmusicians) in the age range between 15 and 30 years old. The stimuli had 5 harmonics, all at the same amplitude (f0 = 330 Hz, mi4). The third (variable tone) has a similar harmonic structure; however, the amplitude of the third harmonic component was higher, producing a different timbre in comparison with the standards. The subject had to identify the odd timbre tone. The testing was performed at 60 dB HL in a sound-treated room. Results The results of the study showed that the profile analysis thresholds were significantly better in musicians compared with nonmusicians. The result of the study also showed that the profile analysis thresholds were better with an increase in the duration of music training. Thus, improved auditory processing in musicians could have resulted in a better profile analysis threshold. Conclusions Auditory stream segregation was found to be better in musicians compared with nonmusicians, and the performance improved with an increase in several years of training. However, further studies are essential on a larger group with more variables for validation of the results.

ABSTRACT

We investigated the mechanisms involved in the relationship between land-use changes and aquatic biodiversity, using stream fish assemblages of the Brazilian Savanna (i.e., Cerrado) as a study model. We tested the prediction that landscape degradation would decrease environmental heterogeneity and change predominant physical-habitat types, which in turn would decrease the functional diversity and alter the functional identity of fish assemblages. We sampled fish from 40 streams in the Upper Paraná River basin, and assessed catchment and instream conditions. We then conducted an ecomorphological analysis to functionally characterize all species (36) and quantify different facets of the functional structure of assemblages. We detected multiple pathways of the impacts from landscape changes on the fish assemblages. Catchment degradation reduced the stream-bed complexity and the heterogeneity of canopy shading, decreasing assemblage functional specialization and divergence. Landscape changes also reduced the water volume and the amount of large rocks in streams, resulting in decreased abundances of species with large bodies and with morphological traits that favor swimming in the water column. We conclude that land-use intensification caused significant changes in aquatic biodiversity in the Cerrado, reinforcing the need to pay special attention to this global hotspot.(AU)


Investigamos os mecanismos envolvidos na relação entre mudanças de uso da terra e biodiversidade aquática, utilizando a ictiofauna de riachos do Cerrado como modelo de estudo. Testamos a predição de que a degradação da paisagem reduz a heterogeneidade ambiental e muda os tipos predominantes de habitat, por sua vez, diminuindo a diversidade e alterando a identidade funcional de comunidades de peixes. Amostramos 40 riachos da bacia do Alto Rio Paraná, e avaliamos as condições da drenagem e do habitat físico local. Em seguida, conduzimos uma análise ecomorfológica para caracterizar funcionalmente todas as espécies (36) e quantificar diferentes facetas da estrutura funcional das comunidades. Detectamos múltiplos caminhos de impacto das alterações da paisagem sobre a ictiofauna. A degradação das bacias de drenagem reduziu complexidade do leito e heterogeneidade no sombreamento pelo dossel, diminuindo especialização e divergência funcional das comunidades. Alterações na paisagem também reduziram volume de água e quantidade de pedras grandes nos riachos, resultando em diminuição na abundância de espécies de maior porte e com atributos morfológicos que favorecem a natação na coluna d'água. Concluímos que a intensificação dos usos da terra causa alterações significativas para a biodiversidade aquática no Cerrado, reforçando a necessidade de especial atenção a este hotspot global.(AU)

Abstract: The length-weight relationship (LWR) and diet of a small trichomycterid catfish, Cambeva guareiensis, was studied based on 58 specimens captured in Corrente stream, Guareí River basin, São Paulo, Brazil. LWR analyses showed values of a ranged from 0.0097 to 0.0149 and b from 2.858 to 3.250. The analysis of the diet showed that high amounts of aquatic insects (93.75% of total items) were consumed by the species. In addition, we found no statistically significant differences when comparing the diet between the dry and rainy periods. Our study contributes to the knowledge on populational biology and feeding ecology of trichomycterid fishes in Brazilian streams.


Resumo: A relação peso-comprimento (RPC) e dieta de um pequeno bagre trichomycterídeo, Cambeva guareiensis, foi estudada com base em 58 espécimes capturados no riacho Corrente, bacia do rio Guareí, São Paulo, Brasil. As análises de RPC mostraram valores de a variando entre 0,0097-0,0149 e de b entre 2,858-3,250. A análise da dieta mostrou que altas quantidades de insetos aquáticos (93,75% do total de itens) foram consumidas pela espécie. Além disso, não encontramos diferenças estatisticamente significativas quando comparamos a dieta entre os períodos seco e chuvoso. Nosso estudo contribui para o conhecimento da biologia populacional e ecologia alimentar de peixes trichomicterídeos em riachos brasileiros.

In recent years, driven by metagenomics sequencing technology, it has been found that compositional changes and diversity changes of gut microbiota are related to the occurrence of hematological malignancies. Gut microbiota affects the development of the hematopoietic system and the risk stratification of hematological malignancies. Microbial translocation plays an important role in and blood stream infection. Gut microbiota imbalance and blood stream infection affect the therapeutic effect of hematological malignancies. Probiotics and fecal microbiota transplant can restore the diversity of gut microbiota.

OBJECTIVE@#To observe the effect of acupuncture at @*METHODS@#Sixty patients with type-2 diabetic peripheral neuropathy were randomly divided into an observation group and a control group, 30 cases in each one. Both groups were treated with basic treatment, and the observation group was additionally treated with acupuncture at Neiting (ST 44), Xiangu (ST 43), Dadu (SP 2), Taibai (SP 3), Zusanli (ST 36), etc. once every other day, 3 times a week for 4 weeks. The changes of TCM symptom score, Toronto clinical assessment (TCSS) score, visual analogue scale (VAS) score of pain and serum tumor necrosis factor α(TNF-α) level were observed before and after treatment in the two groups, and the clinical effects of the two groups were evaluated.@*RESULTS@#Compared before treatment, the TCM syndrome score and the TCSS score in the two groups were reduced after treatment (@*CONCLUSION@#Acupuncture at
